<p>Among women who took misoprostol at 12-hour intervals, pregnancy<br />
continued in 9 percent of those who took the drug orally and in 4 percent<br />
of those who took it vaginally. Among those who took the drug at<br />
three-hour intervals, pregnancy continued in 6 percent of those who took<br />
it orally and in 4 percent of those who took it vaginally.</p>

<p>Side effects such as nausea and vomiting were more common among women<br />
who took misoprostol under the tongue and vaginally at three-hour<br />
intervals than among those who took the drug at 12-hour intervals. Side<br />
effects such as pain, diarrhea, chills and shivering were slightly higher<br />
among women who took the drug orally.</p>

<p>&#8220;Administration interval can be chosen between three hours and 12 hours<br />
when misoprostol is given vaginally. If administration is oral, the<br />
intervals between misoprostol doses need to be short, but side effects are<br />
then increased. With 12-hour intervals, vaginal route should be used,<br />
whereas with three-hour intervals either route could be chosen,&#8221; the study<br />
authors concluded.</p>

researcher Dr. John E. Wagner, director of the bone marrow transplant<br />
program at the University of Minnesota.</p>
</p>
<p>His team published its findings in the June 9 issue of <i>The<br />
Lancet</i>.</p>
</p>
<p>One way to treat various leukemias is to wipe out the old blood cell<br />
system and replace it with a new one via transplant. This study of 785<br />
children under 16 who got such transplants found essentially equal<br />
five-year survival rates for the 503 patients who underwent<br />
transplantation with umbilical cord blood and the 282 who got bone marrow<br />
transplants.</p>
</p>
<p>That held true even when the transplanted blood cord cells did<br />
<i>not</i> match the immunological system of the recipients, a mismatch<br />
that can lead to severe side effects or rejection of the transplant,<br />
Wagner said.</p>
</p>
<p>&#8220;With bone marrow, you have to match eight antigens,&#8221; he said. &#8220;In cord<br />
blood, you need to look at only six, and only need four matches.&#8221;</p>
</p>
<p>Indeed, five-year survival was identical for recipients of perfectly<br />
matched bone marrow and cord blood recipients with two mismatches, Wagner<br />
said. That matching flexibility could expand the field considerably,<br />
because right now bone marrow matches cannot be found for half of all<br />
leukemia patients who need transplants, he said.</p>
</p>
<p>&#8220;With cord blood you can find a match for most patients, where with<br />
marrow you cannot,&#8221; Wagner said. &#8220;The take-home message is that when cord<br />
blood is so promising, it should have an impact on public policy.&#8221;</p>
</p>
<p>And yet the system for extracting and storing cord blood cells still<br />
has many gaps, he said. For example, &#8220;there is no cord banking effort&#8221;<br />
in Wagner&#39;s home city of <a href="http://www.aqua-blogs.com/2008/03/28/caring-for-grandkids-wont-strain-grandparents-health/">Medicine news<br />
</a>, he said.</p>
</p>
<p>Cord blood transplants, originally done only in children, now are<br />
performed routinely for adults with leukemia, Wagner added. &#8220;Of the 8,000<br />
transplants done to date, half have been in adults,&#8221; he said. &#8220;Our next<br />
study will be to look at the outcome in adults.&#8221;</p>
</p>
<p>Other studies also are needed, Wagner said, including those that<br />
compare the cost of the two types of transplant. &#8220;There is no economic<br />
analysis comparing bone marrow and cord blood transplants,&#8221; he said. &#8220;We<br />
also need to look at the quality of life.&#8221;</p>
</p>
<p>The federal government is financing a major effort to expand cord blood<br />
banking, said Dr. Pablo Rubinstein, director of the New York Blood Bank,<br />
the first in this country to store cord blood. The center, which<br />
participated in this study, started cord blood banking back in 1993 and<br />
now collects 8,000 units a year. It currently has an inventory of 40,000<br />
units. The New York Blood Bank has provided material for more than 2,500<br />
transplants, Rubinstein said.</p>
</p>
<p>The system being financed by the federal government &#8220;will have a<br />
national point of access, where all transplant physicians can go to<br />
fulfill their needs,&#8221; he said.</p>
</p>
<p>There also is commercial activity, with some for-profit organizations<br />
urging parents to pay to store cord blood from their children&#39;s births,<br />
Rubinstein said. However, such storage &#8220;is extremely unlikely to be<br />
useful,&#8221; he said, since if a child is found to have a blood problem,<br />
physicians would hesitate to use cells that might well be the source of<br />
that problem.</p>

<p>But they say it&#39;s not clear that these stem cells actually help cancer<br />
develop.</p>
</p>
<p>Many scientists believe that the bone marrow stem cells do boost the<br />
growth and spread of cancer. But the University of Florida researchers say<br />
the stem cells may simply resemble the surrounding cancer cells, but not<br />
act like them.</p>
</p>
<p>The bone marrow stem cells &#8220;have the same kind of surface proteins&#8221; as<br />
cancer cells, noted study author Dr. Chris Cogle, an assistant professor<br />
of medicine at UF&#39;s College of Medicine Program in Stem Cell Biology and<br />
Regenerative Medicine.</p>
</p>
<p>But while the stem cells have the &#8220;same skin&#8221; as the surrounding cancer<br />
cells, the question is whether &#8220;they have the same guts,&#8221; Cogle said.</p>
</p>
<p>&#8220;Our results indicate these cells act as developmental mimics; they<br />
come in and look like the surrounding neoplastic tissue, but they aren&#39;t<br />
actually the seed of cancer,&#8221; Cogle said in a prepared statement.</p>
</p>
<p>&#8220;At the worst, these cells could help support cancerous tissue by<br />
providing it with growth factors or proteins that help the cancer grow and<br />
survive. At the very least, these marrow cells are just being tricked into<br />
coming into the cancerous <a href="http://www.aqua-blogs.com/2008/03/20/chewing-the-fad/">emergency medicine news<br />
</a> and then made to walk and talk like<br />
they don&#39;t usually do,&#8221; the researcher said.</p>
</p>
<p>The study will be published in the August issue of urnal <i>Stem<br />
Cells</i>.</p>
</p>
<p>Cogle noted that about five percent of cancerous tissue contains<br />
marrow-derived cells that resemble surrounding cancer cells. This<br />
&#8220;developmental mimicry&#8221; could affect the results of tests of new drugs on<br />
malignant tissue grown in the laboratory.</p>
</p>
<p>&#8220;If there are bone marrow cells in this cancerous tissue, these (bone<br />
marrow stem) cells may actually contaminate our cancer studies and could<br />
make a difference as to whether or not investigational drugs we&#39;re testing<br />
work or don&#39;t work,&#8221; Cogle said.</p>
</p>

(PET) accurately identifies positive metabolic responses to chemotherapy<br />
in patients with esophageal cancer and helps doctors tailor treatment,<br />
says a German study. </p>

<p>&#8220;This is the first study to apply PET results from early metabolic<br />
response assessment to clinical <a href="http://www.aqua-blogs.com/2008/04/21/eye-protein-could-shield-against-ms/">emergency medicine news<br />

solid tumors,&#8221; Ken Herrmann, a resident in the department of nuclear<br />
medicine at Technical University in Munich, said in a prepared<br />

<p>In the United States, more than 14,000 people are expected to die from<br />
esophageal cancer this year, and more than 15,000 new cases will be<br />
diagnosed, according to background information in a news release about the<br />
study.</p>

<p>The study was presented this week at the Society of Nuclear Medicine&#39;s<br />
annual meeting, in Washington, D.C.</p>

<p>Lyme disease is an infectious disease caused by bacteria carried by<br />
ticks. The nervous system is affected in about 10 percent to 15 percent of<br />
patients infected with Lyme disease.</p>
</p>
<p>&#8220;While other guidelines exist to help diagnose and treat general Lyme<br />
disease, there continues to be considerable controversy and uncertainty<br />
about the best approach to treating neuroborreliosis, in which Lyme<br />
disease involves the nervous system,&#8221; the lead author of the new<br />
guidelines, Dr. John J. Halperin of Atlantic Health in Summit, N.J., said<br />
in a prepared statement.</p>
</p>
<p>He and his colleagues analyzed all available scientific studies on the<br />
topic and found that the <a href="http://www.aqua-blogs.com/2008/03/26/health-tip-paternal-bonding/">health<br />
</a> antibiotics penicillin, <a href="http://www.aqua-blogs.com/2008/03/10/coffee-a-good-source-of-antioxidants/">news in medicine</a>,<br />
cefotaxime, and the oral antibiotic doxycycline are likely safe and<br />
effective when taken for 14 to 28 days by children and adults.</p>
</p>
<p>While other oral antibiotics, such as <a href="http://www.aqua-blogs.com/2008/03/19/drug-firm-plays-up-long-flights-fear/">internal medicine news<br />
</a> and cefuroxime<br />
axetil, may also be safe and effective, there is not enough evidence to<br />
recommend their use, the guideline authors said.</p>
</p>
<p>They also concluded that long-term use of antibiotics does not benefit<br />
Lyme disease patients who continue to have symptoms after receiving<br />
standard antibiotic treatment.</p>

<p>The guideline was published in the May 23 online issue of the journal<br />
<i>Neurology</i>.</p>

<p>&#8220;The new finding is that in men who have risk factors for heart attack,<br />
even six months of <a href="http://www.aqua-blogs.com/2008/02/28/malaysian-muslims-told-not-to-use-botox/">medicine hat news paper<br />

years,&#8221; said lead researcher Dr. Anthony D&#39;Amico, chief of genitourinary<br />

Women&#39;s Hospital in Boston.</p>
</p>
<p>That finding, which comes from analysis of pooled data of studies in<br />
the United States, Australia and New Zealand, does not mean that such men<br />
should not be treated to suppress the activity of androgens &#8212; male sex<br />
hormones that spur the growth of prostate cancer cells, D&#39;Amico said.</p>
</p>
<p>Instead, &#8220;the implication is that a man who needs hormonal therapy to<br />
avoid dying from cancer but also has risk factors for heart attack should<br />
be sent to a cardiologist for assessment and possible treatment of heart<br />
disease before starting hormonal therapy,&#8221; he said.</p>
</p>
<p>&#8220;We&#39;re doing that,&#8221; D&#39;Amico said. He noted that, &#8220;of about 50 men we<br />
referred in the last six months, five or six had <a href="http://www.aqua-blogs.com/2008/03/19/slimmer-sues-atkins-over-cholesterol/">medicine hat news obituary<br />
</a> coronary<br />
artery disease. They have had it treated and have gone through hormonal<br />
therapy without being affected.&#8221; </p>
</p>
<p>The findings are published in the June 10 issue of the <i>Journal of<br />
Clinical Oncology.</i></p>
</p>
<p>Androgen suppression therapy (AST), as it is formally called, is<br />
reserved for men whose cancer is believed to have spread beyond the<br />
prostate or who have an aggressive form that is believed to have spread &#8211;<br />
something that occurs in perhaps 40 percent of cases, D&#39;Amico said. &#8220;You<br />
give these drugs to starve the prostate cancer, and it dies,&#8221; he said.</p>
</p>
<p>Other side effects of AST are well-known. It can cause anemia, increase<br />
body fat, reduce muscle and cause an increase in harmful LDL (&#8221;bad&#8221;)<br />
cholesterol and a decrease in helpful HDL (&#8221;good&#8221;) cholesterol. But AST is<br />
also widely used, because it extends prostate cancer survival.</p>
</p>
<p>The new <a href="http://www.aqua-blogs.com/2008/04/01/bone-marrow-may-give-rise-to-blood-cancers/">medicine news<br />
</a> on the treatment&#39;s adverse cardiac effects comes<br />
from analysis of data from 1,372 men who received radiation treatment plus<br />
AST in three randomized trials and who were followed for at least five<br />
years.</p>
</p>
<p>A faster onset of heart attacks was observed in men over 65 who got AST<br />
for six months, the researchers found.</p>
</p>
<p>&#8220;This is one more reason to be careful when you recommend hormone<br />
therapy,&#8221; said Dr. Eric M. Horwitz, clinical director of radiation<br />
oncology at Fox Chase Cancer Center in Philadelphia. &#8220;They do have cardiac<br />
side effects.&#8221;</p>
</p>
<p>But AST will continue to be used in many cases, he added. &#8220;There is<br />
clearly a group of men where the benefits outweigh the risks,&#8221; he said.<br />
&#8220;This study shows that you have to weigh the pros and cons of the<br />
treatment, who gets benefits and who doesn&#39;t.&#8221;</p>
</p>
<p>Men who benefit most are those with &#8220;aggressive, bulky prostate<br />
cancers,&#8221; according to Horwitz. &#8220;The benefits for them still outweigh the<br />
risk in terms of trying to cure the cancer. For men with less aggressive,<br />
less bulky cancers, you have to weigh the benefits versus the risks.&#8221;</p>
</p>
<p>Other efforts to get the best effect from AST while minimizing the<br />
damage are under way, D&#39;Amico said. One method under study is to stop the<br />
treatment now and then. &#8220;For advanced prostate cancer, we don&#39;t expect to<br />
keep the treatment going forever,&#8221; he said. &#8220;You can make it intermittent<br />
to get the same survival with less toxicity &#8212; six months on and six<br />
months off,&#8221; he said.</p>
</p>
<p>Also, some studies indicate that a shorter course of AST can be<br />
effective with fewer side effects &#8212; at least for some patients, D&#39;Amico<br />
said. &#8220;We need to be better at selecting men for therapy and directing its<br />
course,&#8221; he said.</p>
